From Administration
- Service Record - This form begins the process for setting up pension payments
- Application to Begin Benefits - To be signed by your present employer confirming your retirement plans. (You may wait to send this form if you wish, until you have informed your church.) - If you are no longer serving in a Covenant institution, you do not need to have the bottom half of this form signed.
- Direct Deposit - For requesting direct electronic deposit of your monthly benefit check into your bank account.
- Surviving Spouse - For choosing 75% or 100% for your surviving spouse. Must be elected no less than two years before normal or early retirement date.
